What creatine actually does for women

Dr. Abbie Smith-Ryan runs one of the few labs in the world that studies creatine specifically in women, at the University of North Carolina. Here is how she opens the subject:

Creatine is not a stimulant, a hormone, or a drug. It is a compound your cells use to make and recycle energy. Your body makes some. You eat the rest, mostly in red meat and fish.

The reason this is a women’s question and not a gym question is the starting line. Women carry substantially lower creatine stores than men, and on average eat less of it. A women’s-health review across the lifespan puts the gap at 70 to 80 percent lower stores in women than in men. You are not adding something foreign to your body. Why it lands differently past 40

Somewhere in the mid-40s, the same workout, the same effort, and the same protein start returning less. That is not in your head, and it is not a discipline problem. As estrogen shifts through perimenopause, muscle gets harder to build and harder to hold, recovery slows, and energy runs closer to the line.

The evidence backs her. A 2026 meta-analysis pooled 7 randomized controlled trials in 608 postmenopausal women, average age about 62. Creatine at 5 grams a day, paired with resistance training, beat training alone: about 0.37 kg more lean mass and about 7.5 kg more leg-press strength.

Two notes, because we would rather you trust us than be impressed. Those women were postmenopausal. The reasoning starts earlier, in perimenopause, but the cleanest data sits further along. And every bit of that gain came with training. Hold that thought.

What creatine will not do

This is the section that matters, because every line in it is something a woman has been told about her own body that is not true.

It will not make you bulky. Take it from the researcher, speaking to the supplement industry about its own habit of selling women pink tubs:

“We don’t need pink products, and creatine isn’t gonna build a booty.”

The mechanism, since “don’t worry” is not an argument: visible size takes a sustained calorie surplus and a hormonal profile most women simply do not have. Creatine supports the energy in the muscle you already train. It does not manufacture bulk. Women who take it get stronger, not bigger.

The water-weight question, straight. Creatine pulls a little water inside the muscle cell. That is part of how it works, and it is not the puffy under-skin kind. The amount is also far smaller than you have been told. Smith-Ryan: “In most studies, we’ve seen about a half a pound. And honestly, we see it less in women. Our most recent paper shows half a pound.”

Half a pound, mostly from “loading” with high doses for a week, and it settles within days. Skip loading, as we do, and it is smaller still. Much of what gets blamed on creatine is actually the sugar alcohols and sweeteners it was mixed with.

There is a second half nobody mentions. Smith-Ryan’s lab found creatine pulled fluid out of the extracellular space and into the muscle in the luteal phase, easing the swelling many women get before their period. For a lot of women, the real effect is the opposite of the fear.

It will not ruin your kidneys. This one is a lab misunderstanding that got loose and never got corrected. Creatine raises serum creatinine, a marker on a blood panel, without changing the kidney’s actual filtration in healthy people. The number on the test moves. The kidney does not. It is worth telling your doctor you take creatine before a kidney panel, so the marker is not misread. If you already have a kidney condition, ask your clinician first.

It will not make your hair fall out. The entire hair scare rests on a single 2009 study in college rugby players that measured a hormone and never once measured hair. The first trial to actually look at hair, in 2025, found no difference from placebo in DHT or in hair growth. A claim about women’s hair, built from a study of men that never looked at hair.

It is not a weight-loss product. Creatine does not burn fat. If a page is selling it to you that way, close the tab.

How much, and how
